Discussed social and family history: patient is unmarried and had been a moderate drinker. Discussed patient’s present condition: patient was well, 5ft and 6 ½ inches, and ten stones. Discussed abdominal [illegible]: patient was frequently thirsty, had a good appetite, and regular bowels. Treatment detailed [illegible]. Three days later on November 12, patient felt better, passed 68 ounces of urine, recorded high albumin. Notes listed subsequent dates with urine quantity, albumin levels, and weight. |
