Four of the forms give the name of the practitioner; age; birth date; marital status; address; address at date of national registration (15th August 1915); occupation; occupation before 15th August 1915; name and address of employer; grounds on which application made from a list of five; reasons in support of the application; any previous application made; and name and address of applicant.
The fifth form is for Voluntarily Attested Men and gives the name of the practitioner; age; birth date; marital status; date of marriage; address; address at date of national registration (15th August 1915); where attested; date of attestation; occupation; occupation before 15th August 1915; name and address of employer; grounds on which application made from a list of five; reasons in support of the application; any previous application made; name and address of applicant; and the decision of the tribunal. Also includes two lists of practitioners and the times of their hearings. |
