The forms give the practitioner's name and permanent address; theatre of war, period served and branch of service; date of birth; marital status; rank; and details of practice including location, whether single handed or in partnership etc. If a practitioner had additional details that were not covered in the form they were invited to send these in a letter to the Ministry some of which are attached to the forms. Some forms also have an additional form attached which was used to calculate eligibility for demobilisation. Section A on the form was divided into those born before November 1888 and those born after with additional points if any to be awarded by the Central Medical War Committee. Section B was for the calculation of points according to period of service, date of birth, domestic situation and practice details. |
