The first pages give the name and address of the practitioner; place; division; date of application; name and address of applicant (if different); grounds of application; date considered by Committee; Committee's decision; and number of the letter of excuse. Dated 25th January 1916-16th February 1916, the entries are arranged by date of application. After this are entries by doctor's name and whether granted or refused 26th July 1916-24th October 1918. Lastly are exemptions by division and year 1916-1918; and the difference in the number of doctors between 1914 and 1918 by class and geographical division. |
