Description Of Item | The forms were issued by the Scottish Medical Service Emergency Committee. The form was required under the Military Service (Professional Committees Regulations) Order, 1916. The form gives the name of the practitioner; age last birthday; address; qualifications; body to which notice given of application (eg Local War Committee); name of institution or person by whom application is made; reasons for application; documents in support of the application; and whether the applicant wants to be seen by the Committee. Most applications include supporting documentation and correspondence.
There are three sets of applications. The main set was consecutively numbered 1-52; a second set has been annotated with the times that the applicant was to be seen by the Committee but have not been numbered; and there are four applications which were found in a separate packet.
