Born in Edinburgh, James Carmichael was the son of William Carmichael, MD, FRCS. He was educated at the Edinburgh Institution. He was one of the last of those who graduated with a MD immediately on completion of the medical course. As a student he was President of the Royal Medical Society. He became a Fellow of the Royal College of Physicians of Edinburgh in 1875. After graduation in 1864, he practised in Burntisland, Fife, returning to Edinburgh in 1873, having decided to specialise in diseases of children. His ambition was achieved by his appointment as Physician to the Royal Edinburgh Hospital for Sick Children.

Dr Carmichael took an interest in charitable institutions for children and for many years was Chairman of the Society for Prevention of Cruelty to Children. He was a member of the League of Pity Committee and of Children's Shelter. He was a member of the Royal Company of Archers and won the King's Prize on two occasions.
